Why a proper case history changes everything

If you have only ever had a 10-minute appointment, the idea of a 90-minute first consultation can feel excessive. It is not. It is the foundation everything else is built on, and it is usually the difference between a generic plan and one that actually works for you.

There is no “what to eat” without “who you are”. Without the case history, any plan is a guess in expensive packaging.

Symptoms become clues in context

Bloating that started after antibiotics is a different conversation from bloating that started in perimenopause. Fatigue that follows long-COVID is a different conversation from fatigue that has been there since childhood. The case history sets the context that turns a symptom into a clue.

Patterns over single data points

One symptom in isolation is rarely the whole picture. The combination of symptoms, the order they appeared in, what changed in the months before, the response to specific foods, and the link with stress and the menstrual cycle is what allows real pattern recognition. The pattern is the diagnosis.

What the first consultation covers

The presenting concern in detail. Medical and family history. Lifestyle, sleep, stress and movement. Diet and the relationship with food. Bowel and menstrual patterns. Past treatments, previous test results, current medication and supplements. Where appropriate, iridology, tongue and nail analysis. You leave with initial recommendations and a clear sense of what we will do next.

How this saves on testing

A good case history dramatically reduces the amount of testing we need to do. Many cases are clear enough from the history that we can start the plan immediately. Where testing is useful, the history tells us exactly which test will be most informative. Tests follow the case, not the other way around.

About Caroline Peyton

Naturopath, Nutritionist and Gut Health Expert

I qualified with a Diploma in Naturopathic Nutritional Therapy in 2009 and have been in clinical practice ever since. My particular passion is gut health and the microbiome, because, as naturopaths have said for decades, true health really does start in the gut.

Along the way I helped launch the Naturopathic Nutrition Association and chaired it for seven years, ran the Natural Healthcare College as Principal for five years, and in June 2024 I was invited onto the Board of the General Naturopathic Council, the UK regulator for Naturopathy.

Where is your clinic for Faringdon clients?

The Swindon clinic is at Wood Street Wellbeing, 10 Wood Street, Swindon, SN1 4AB, in Old Town Swindon, a short drive down the A420 from Faringdon. Do I have to come in person, or can I see you online?

Both work well. Many Faringdon clients choose in person for the first consultation so we can include iridology, tongue and nail analysis, and then run follow-ups by Zoom. If travel is awkward, the entire programme can run online from the start.

What does a typical first consultation look like?

The first consultation runs for around 90 minutes. We work through a detailed naturopathic case history covering your symptoms, medical history, lifestyle, diet, sleep, stress and family history. Where appropriate we include iridology, tongue and nail analysis. You leave with initial recommendations and a clear sense of what we will do next.

How is your approach different from the NHS?

The NHS does an excellent job of acute care and ruling out serious disease. Where it has less time and capacity is for the slow, complex, multi-symptom cases that need a long case history and a personalised plan. That is exactly the gap a naturopathic nutritionist fills. We work alongside, not against, your GP.

Can I bring previous test results?

Yes, please do. Recent blood tests, scope reports, microbiome tests, food sensitivity panels or anything else relevant. The more context I have, the less likely it is that we run tests you do not need.

Do I need a referral from my GP?

No. You can book directly. If you would like me to liaise with your GP at any point, I am happy to write to them with your consent.

What is the GNC and why does it matter?

The General Naturopathic Council is the regulator for Naturopathy in the UK. As a Board Member, I help shape the professional standards that any naturopath in the UK should be held to. For you as a client, it means working with someone whose accountability is public and verifiable.
